Our client is dedicated to enhancing care in the community, assisting individuals to remain healthy and independent in their homes. This role is situated within the Continuing Care services of the Edmonton Zone, covering various program areas.

Position Title: Transition Coordinator (TC)

The Transition Coordinator is a clinical consult-based position responsible for assessing patients’ needs holistically and determining suitable continuing care options. This role involves a high level of complexity, requiring continuous communication with other healthcare providers.

Major Responsibilities

  • Conducting comprehensive electronic needs assessments.
  • Determining urgency and priority of care.
  • Developing and implementing service plans.
  • Coordinating service delivery across various programs and settings.
  • Liaising with Continuing Care staff and community resources.
  • Providing information and support to clients, families, and caregivers.
  • Participating in multi-disciplinary team meetings and evaluations.

    • Position Description

    As a Registered Nurse (RN), you will deliver diverse nursing services to patients and families while ensuring their safety and well-being. Your role will involve utilizing critical thinking and problem-solving skills, as well as advocating for patient needs.

    Compensation

  • Provisional Permit Holders: $39.58 /hour – $54.16/hour.
  • Pay adjustments upon registration with the College of Registered Nurses of Alberta (CRNA).

    • Role Requirements

  • Completion of an accredited nursing education program.
  • Active or eligible registration with the CRNA.
  • Current Basic Cardiac Life Support – Health Care Provider certification.
  • Minimum five years facility-based nursing experience, including recent geriatric experience.
  • Experience in community nursing and home care.

    • Preferred Qualifications

  • Proficiency in computer applications like Microsoft Word and specific healthcare tools.

    • Working Conditions

  • Must possess a driver’s license and a vehicle.
  • Ability to cope with high stress and time-sensitive deadlines.
  • Physical ability to perform hands-on care and manage patient transfers.
