This Transition Coordinator position is clinical consult based and located at a specific hospital where you will be a part of a team of Transition Coordinators with an assigned portfolio/inpatient unit(s) including Emergency. As a TC, you will assess patients’ needs in the context of the whole person, including their life situation and determinants of health. Critical thinking and accurate interpretation of complex information from a variety of sources are essential to the clinical decision-making required for determining appropriate continuing care living options. The Transition Coordinator role is highly complex and includes comprehensive assessment, planning and coordination of care, as well as ongoing communication with other healthcare providers across the continuum of care. Upon hire, you will receive specialized training providing you with the knowledge, tools, and resources to confidently care for your patients. Major responsibilities include: completing comprehensive electronic needs assessments, determining urgency/priority, developing and implementing appropriate service plans, coordinating service delivery across programs/settings, liaising with Continuing Care staff and other community resources, providing information and support to clients, families, and caregivers. The TC works within a multi-disciplinary team of health professionals and is responsible for appropriate referral to other programs/services, appropriate documentation, participation in conferences and team meetings, process, and outcome evaluation and professional development activities. Applicants should be aware that there are physical demands/repetitive movements related to patient assessment and coordination.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes through critical thinking, problem solving and decision-making, as well as teaching, counseling, and advocating on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance. You will provide safe, quality patient and family-centered care while reflecting shared vision and values.
Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the relevant nursing college. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of a series of required courses is necessary.
Additional Required Qualifications:
A minimum of five years of facility-based nursing experience, including geriatric experience. A minimum of two years’ experience within community nursing (home care, supportive living, or facility living). Effective time management and prioritization skills, conflict resolution, and strong communication skills. Must be able to work independently and within a team environment. Ability to problem-solve and demonstrate critical thinking skills. Must provide references that can substantiate the above qualifications.
Preferred Qualifications:
Knowledge of computer applications such as Microsoft Word and healthcare management tools is an asset. Active Transition Coordinators with up-to-date certification may get preference.
