The Diabetes RN provides expert nursing care, education, and consultative services to patients and families living with diabetes. Supporting patients during clinic visits and through telephone/virtual interactions, the RN applies specialized knowledge and clinical expertise in diabetes education and management. This role plays a critical part in delivering comprehensive, patient-centered care within an ambulatory setting, working as a key member of an interdisciplinary team. The Diabetes RN is responsible for assessing, triaging, and coordinating care for patients in the diabetes program, utilizing clinical judgment, critical thinking, and the nursing process. Acting as a liaison between the interdisciplinary care team and external providers, the RN ensures continuity of care and supports patient self-management and education. In addition to diabetes-specific responsibilities, the RN may provide education to non-diabetic endocrinology patients and hospitalized patients requiring diabetes support.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving, and decision making, as well as teach, counsel, and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance. You will provide safe, quality patient and family-centered care while reflecting the shared vision and values of our client.
PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour – $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse.
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of specific courses is required.
The candidate must have their Certified Diabetes Educator (CDE) certification, with demonstrated expertise in diabetes education and management, covering insulin and medication therapy, blood glucose monitoring, and comprehensive care planning. Experience in ambulatory care nursing, strong interpersonal and communication skills, and a patient-centered approach are essential. Proficiency with electronic health records and Microsoft Office is required. The candidate must demonstrate strong documentation abilities, work effectively both independently and within an interdisciplinary team, and possess excellent organizational and prioritization skills.
This position involves moderate physical activity, including frequent overhead reaching, upward looking, and prolonged computer use. The successful candidate will assist patients with mobility challenges, which may involve lifting, supporting clients, and tasks requiring bending and walking long distances. Regular lifting and carrying of 10 to 15 kilograms is expected, and the candidate must be able to lift up to 50 pounds.
