Our client is dedicated to enhancing care in the community, helping individuals maintain health, wellness, and independence in their homes and communities. The Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in clients’ homes across all ages and stages of life.

As a self-directed team member within Continuing Care in the Edmonton zone, the Registered Nurse will collaborate with the healthcare team and have responsibilities that include:

• Assessing needs and delivering appropriate therapeutic nursing services within professional practice standards.
• Coordinating resources and establishing goals for client delivery.
• Evaluating outcomes and providing case management as well as direct client care.
• Acting as a consultant on the team, providing clinical expertise and direction.
• Developing care plans based on unmet needs using appropriate resources.
• Participating in staff development and mentoring student clinical experiences.

Description

As a Registered Nurse (RN), you will provide a wide array of nursing services, ensuring patient safety and well-being. Your role involves:

• Utilizing nursing processes through critical thinking, problem solving, and decision-making.
• Teaching, counseling, and advocating on behalf of patients and their families.
• Supporting an integrated and holistic approach to patient care, health promotion, and maintenance.
• Providing safe, quality, client-centered care that reflects the shared vision and values of our client.

Please note: Provisional Permit Holders will be paid $39.58/hour – $54.16/hour. Upon registration as a Registered Nurse, the pay rate will adjust according to applicable agreements.

Required Qualifications

• Completion of an accredited nursing education program.
• Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
• Completion of specific online courses if you have not practiced as a Registered Nurse within the last five years.

Additional Required Qualifications

• Two years of recent nursing experience or completion of a refresher program within 2 years.
• Self-directed with the ability to work independently.
• Excellent organizational, communication, and assessment skills.
• Demonstrated computer knowledge and proficiency.
• Physically capable of performing nursing tasks including client transfers.

Preferred Qualifications

• Baccalaureate degree in nursing preferred.
• Palliative care experience.
• Previous experience in Continuing Care and/or geriatrics.
• Experience with Meditech, Connect Care, and the RAI Home Care assessment tool.
• Two years current Case Management Experience in Continuing Care.
