Our client is dedicated to enhancing care in the community, helping people be healthy, well, and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life.

As a self-directed team member with Continuing Care in the Edmonton zone, the Registered Nurse works collaboratively with the healthcare team. Responsibilities include:

• Assessment of needs
• Delivering appropriate therapeutic nursing services within professional practice standards
• Coordinating resources and establishing goals for client delivery
• Evaluating outcomes
• Providing case management and direct client care that meets established clinical standards, and is evidence-based and client-centered

In collaboration with family and other partners, the Registered Nurse acts as a consultant on the team, providing clinical expertise and direction to other team members regarding client cases, developing care plans based on unmet needs, and utilizing appropriate internal and external resources. As a member of the multidisciplinary team, the Registered Nurse is responsible for participating in staff development and mentoring student clinical experiences.

Description

As a Registered Nurse (RN), you will provide a variety of nursing services to patients, families, communities, and populations, while ensuring their safety and well-being. In your role, you will utilize nursing processes through critical thinking, problem-solving, decision-making, teaching, counseling, and advocating on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance.

You will provide safe, quality patient and family-centered care while reflecting shared vision and values.

Compensation

Provisional Permit Holders shall be paid $39.58/hour – $54.16/hour. Upon entering the register of Registered Nurses, your rate of pay will be adjusted to the applicable rate of pay.

Required Qualifications

• Completion of an accredited nursing education program
• Active or eligible for registration with the College of Registered Nurses of Alberta (CRNA)
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP)
• If you have not practiced as a Registered Nurse in the last five (5) years, completion of specific courses is required

Additional Required Qualifications

• Two years of recent nursing experience (obtained within the last 5 years) or completion of a refresher program within the last 2 years
• Self-directed and ability to work independently
• Excellent organizational skills
• Strong communication and assessment skills
• Physically capable of performing nursing tasks involving client transfers

Preferred Qualifications

• Baccalaureate degree in nursing preferred
• Palliative care experience
• Previous Continuing Care and/or geriatric experience
• Experience with Connect Care and RAI Home Care assessment tool
• Two years current Case Management Experience in Continuing Care
