Your Opportunity:Our client employs a strengths-based Client Centered Case Management practice framework in Integrated Home Care. In this role, you will develop a care plan that helps clients and their families identify needs, focus on strengths, set achievable health and self-care goals, and provide direct nursing care. Working collaboratively with the client/family and interdisciplinary team, you will ensure that services are continuously updated according to the client’s response to interventions while incorporating stewardship into service authorization. Reporting to the Home Care Manager, you’ll be responsible for coordinating and delivering quality care to clients in the community. You will conduct nursing assessments, case coordination, and direct care in partnership with clients and their families in their residences or alternate care settings, as outlined by assessment. The successful candidate will demonstrate resilience in key areas including personal and professional accountability, systems thinking, creative thinking, communication, relationship building, organizational skills, time management, interpersonal skills, and intermediate computer skills.
Description:As a Registered Nurse (RN), you will provide a broad range of nursing services to patients, families, communities, and populations while taking necessary precautions to ensure their safety and well-being. Your responsibilities will include using nursing processes with critical thinking, problem-solving, and decision-making, as well as teaching, counseling, and advocating for patients and their families. You’ll also play a leadership role in promoting an integrated and holistic approach to patient care, health promotion, and maintenance. Providing safe, quality, patient and family-centered care will reflect the shared vision and values of our client.
Required Qualifications:
Completion of an accredited nursing education program. You must be active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support for Health Care Provider (BCLS-HCP) certification is required. If you haven’t practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is necessary: CRNA Medication Management – Chapters 1 – 4 and the NextGenU Health Assessment Resources Certification.
Additional Required Qualifications:
1 – 2 years of recent Home Care experience or a comparable combination of education, training, and experience. Excellent organizational and time management skills are essential. You should have demonstrated critical thinking and decision-making abilities, assessment skills, and proficiency in using an electronic health record. The ability to work independently, along with strong negotiation and conflict resolution skills, is imperative. This role requires functioning effectively in a fast-paced and challenging environment, including travel for work purposes. Manual handling (lifting, lowering, carrying, pushing, and pulling) is a regular part of this job; for example, lifting approximately 30 pounds in and out of a vehicle regularly and navigating stairs is commonplace. Learning resources and training will be provided to ensure safety in your duties.
Preferred Qualifications:
A Bachelor’s degree in Nursing. Experience working with acute clients and/or long-term clients focusing on chronic disease management is preferred. Familiarity with mental health and/or addiction/harm reduction, case management services, Home Parenteral Therapy (HPTP), wound care, ostomy care, palliative support, and client education is beneficial.
