Please note this position is approximately 64 km SW outside of Calgary City Limits and or approximately 80 km SW outside of Lethbridge City Limits. The Licensed Practical Nurse (LPN) in Home Care works as a member of an inter-professional health care team utilizing the nursing process to provide direct nursing care to individuals across the lifespan. The LPN is responsible for their nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse Case Home Care Manager, the LPN assesses, plans, implements, and evaluates the needs of Home Care clients and their caregivers/families.
Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals, and provide direct nursing care. In collaboration with the client/family, you will ensure services are provided and modified on an ongoing basis based on the client’s response to interventions, incorporating stewardship into service authorization. There is also an expectation to be a mobile worker, including caring for clients in their homes or in the community.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem-solving, and decision-making skills. You will be guided by your education, experience, demonstrated skill level, and policies and procedures. You will play a key role in providing safe, quality patient and family-centered care while reflecting the shared vision and values of the organization.
You will be responsible for carrying and moving equipment, using and operating a laptop computer and mobile phone, and manual handling (lifting, lowering, carrying, pushing, and pulling upwards to 50 lbs) as a regular part of the job. Learning resources and training are provided to safely perform work. Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related purposes is a requirement of this position.
