Your Opportunity:The Licensed Practical Nurse – PCU 71 and 72 (LPN) works collaboratively within a multi-disciplinary healthcare team on two 38-bed Acute/Sub Acute Medical Unit(s) at our client’s facility. The patient population on these units includes acute medical patients and patients awaiting transfer to alternative care settings in the community. The Licensed Practical Nurse (LPN) will work collaboratively with a multi-disciplinary team to ensure that our patients receive high-quality, patient/family centered care. These patients have complex and diverse care needs. The LPN will be oriented and required to work on both Unit 71 and Unit 72.
Description:As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem-solving, and decision-making skills. You will be guided by your education, experience, demonstrated skill level, and relevant policies and procedures. You will play a key role in providing safe, quality patient and family-centered care while reflecting the shared vision and values of the organization.
Required Qualifications:Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Current Required Organizational Learning (ROL) is necessary. Recent (within the last 2 years) acute medicine nursing experience and a strong clinical interest in medicine and geriatrics are essential. Demonstrated ability to work and lead in a team environment. Proven ability to work effectively with patients and families in stressful situations. Skills to manage patients with dementia, brain and spinal cord injuries, and those with challenging behaviors are required. Strong clinical and assessment skills necessary (Patient assessment, IV certification, Foley insertion, and Glucometer certification). Excellent interpersonal communication, effective time management, and organizational skills are critical for collaboration with the multidisciplinary team. Ability to work in a fast-paced department that is variable, unpredictable, and has competing priorities. Strong conflict resolution and case management skill set necessary. Ability to communicate clearly and concisely with all members of the healthcare team and with patients and family members. Ability to lift and turn patients safely utilizing appropriate techniques and equipment. Ability to lift and transfer up to 50 lbs. up to 25% of the time – utilizing appropriate equipment and techniques.
Preferred Qualifications:Relevant Senior’s and Geriatric Health education is highly preferred. Connect Care experience is preferred.
