We are dedicated to enhancing care in the community, helping people be healthy, well, and independent in their homes and communities. Our continuing care team provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. As a self-directed team member, the Registered Nurse works collaboratively with the health care team and is responsible for:

• Assessment of needs
• Delivering appropriate therapeutic nursing services within professional practice standards
• Coordinating resources
• Establishing goals for client delivery
• Evaluating outcomes
• Providing case management as well as direct client care that meets established clinical standards, and is evidence-based and client-centered
• Acting as a consultant on the team, providing clinical expertise and direction to other team members regarding client cases
• Developing care plans based on unmet needs using appropriate internal and external resources
• Participating in staff development and mentoring student clinical experiences

Description

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations, ensuring their safety and well-being. In your role, you will utilize nursing processes through critical thinking, problem-solving, and decision-making, as well as teach, counsel, and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance.

You will provide safe, quality patient and family-centered care while reflecting our shared vision and values.

• Maximum Salary: $59.21

Required Qualifications

• Completion of an accredited nursing education program
• Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP)

Additional Required Qualifications

• Two years of recent nursing experience (within the last 5 years) or completion of a refresher program within 2 years
• Self-directed with the ability to work independently and use sound professional judgment
• Excellent organizational skills
• Strong communication and assessment skills
• Physically capable of performing nursing tasks

Preferred Qualifications

• Baccalaureate degree in nursing preferred
• Palliative care experience
• Previous Continuing Care and/or geriatric experience
• Experience with Meditech, Connect Care, and the RAI Home Care assessment tool
• 2 years current Case Management experience in Continuing Care
