You will work with patients who have unique life experiences in a facility that includes 36 long-term and palliative care spaces and 72 supportive living spaces. Description:As an LPN, you will be part of an interprofessional health care team, utilizing critical thinking, problem-solving, and decision-making skills. You will provide safe, quality patient and family-centered care, reflecting the vision and values of our client.

• Minimum Salary: $27.58
• Maximum Salary: $36.13

Required Qualifications:

• Completion of an accredited practical nurse education program
• Active or eligible for registration and practice permit with the College of Licensed Practical Nurses (CLPNA)
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP)

Additional Required Qualifications:

• Knowledge of Medication Assistance Program (MAP)
• Strong communication and leadership skills
• Ability to manage conflicting priorities
• Demonstrated critical thinking and problem-solving skills
• Ability to work independently and as part of a team
• Required to work to full scope of practice of LPN

Preferred Qualifications:

• 1 to 2 years recent experience in long-term care or supportive living
• Understanding of collaborative care and resident-focused care
• Dementia Care Training (e.g., Supportive Pathway, P.I.E.C.E.S)
• Experience with palliative care clients and families
• Advanced foot care training and experience
• Wound care experience
• Experience in a charge role preferred
