Our client is dedicated to Enhancing Care in the Community, helping people be healthy, well, and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life.

Position Overview:

As a self-directed team member with Continuing Care, the Registered Nurse demonstrates strong team player skills and has responsibilities, including:

• Assessment of needs
• Delivering appropriate therapeutic nursing services within professional practice standards
• Coordinating resources and establishing goals for client delivery
• Evaluating outcomes
• Case management of clients

This position involves direct provision of client care that meets established clinical standards, which is evidence-based and client-centered, and acts as a consultant on the team providing clinical expertise and direction to other team members regarding their cases. In collaboration with the client and case manager, the direct RN care plans to address client’s unmet nursing needs using appropriate internal and external resources through nursing assessment and interventions to clients. This position is integral to the Short-Term Intervention Team (STIT), which provides triage and clinic support, as well as community visits that require specialized skills in cardiac, IV therapy, and wound care.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes through critical thinking, problem solving, and decision making, as well as teach, counsel, and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance. You will provide safe, quality patient and family-centered care while reflecting shared vision and values.

Compensation:

Please note that Provisional Permit Holders shall be paid $39.58 /hour – $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate applicable to a Registered Nurse in accordance with relevant guidelines.

Required Qualifications:

• Completion of an accredited nursing education program
• Active or eligible for registration and practice permit with the College of Registered Nurses
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP)
• Excellent organizational and time management skills
• Communication and assessment skills to work cooperatively and effectively with the healthcare team
• Physically able to perform all assigned tasks

Preferred Qualifications:

• Baccalaureate degree in nursing preferred
• Experience in Palliative Care and Connect Care
• Two years current nursing practice with one year in a Continuing Care based setting
• Experience with complex clients including acute, chronic, and end of life conditions
• Experience with cardiac, IV therapy, and wound care
