Your Opportunity

The Registered Nurse (RN) Case Manager collaborates, coordinates, and delivers healthcare services, using expertise in advocacy and a patient- and family-centered approach. You will work with clients of various needs in home settings. The position requires travel up to one hour from your home site to clients’ residences during all seasons and at various times of the day. This role includes physical requirements such as bending, lifting, twisting, walking, climbing stairs, and carrying heavy items. On-call duties are also a part of this position.

Responsibilities

• Assessing, planning, implementing, and evaluating client care with a focus on chronic disease management.
• Providing health assessments, wound care, ADL support, placement in facilities, and palliative care.
• Collaborating with individuals, caregivers, and community resources to support healthy living and community connectedness.
• Utilizing strong interpersonal skills and documentation capabilities.

Location

This position is located in beautiful Fort McMurray, which resides on Treaty 8 territory and the traditional lands of the Metis. Each Indigenous community is diverse in size, culture, and character. Description

As a Registered Nurse (RN), you will provide a variety of nursing services to patients, families, communities, and populations, ensuring their safety and well-being. You will utilize nursing processes through critical thinking and problem-solving, while also teaching, counseling, and advocating on behalf of patients and their families. Play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance.

Compensation

Please note: Provisional Permit Holders will be paid $39.58/hour – $54.16/hour. Rates for Registered Nurses will be adjusted according to the applicable pay scale.

Required Qualifications

• Completion of an accredited nursing education program.
• Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
• If you have not practiced as a Registered Nurse within the last five years, completion of specified free courses is required.

Additional Required Qualifications

• 2 years of bedside nursing experience in the last five years.
• Reliable vehicle and business insurance required.
• Ability to drive during various conditions, including night time and winter conditions.
• Inter RAI HC certification.
• Connect Care experience and experience in supervising unregulated health care staff.
• LEAP certification and experience in wound vac, wound care, ostomy, and continence care.

Preferred Qualifications

• Aids to Daily Living (AADL) authorization certification preferred.
• 1-3 years of Home Care Case Management and wound care experience.
