Your Opportunity:Our client is located along Highway 2 in the Battle River Valley within an hour’s drive to major cities. The home care program serves a large area of clients including the community and surrounding areas. We work closely with community groups, other healthcare services, and as part of a multidisciplinary team. The Registered Nurse – Case Manager is responsible for directing the care of clients under the home care program. The Case Manager must possess strong interpersonal communication and interdisciplinary team-player skills, excellent critical thinking, creativity, resourcefulness, time management, and organizational abilities.

Responsibilities:

• Assessment of client needs
• Establishing goals for client care delivery
• Planning, delivering, and coordination of care needs, resources, and services within professional practice standards
• Communication and coordination of client care with the interdisciplinary team
• Evaluation of outcomes

Description:As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes through critical thinking, problem solving, and decision-making, as well as teach, counsel, and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance. You will provide safe, quality patient and family-centered care while reflecting the shared vision and values of our client.

Compensation:

PLEASE NOTE: Provisional Permit Holders shall be paid $39.58, and upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the applicable rate for Registered Nurses in accordance with relevant articles.

Required Qualifications:

• Completion of an accredited nursing education program
• Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
• Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP)
• If you have not practiced as a Registered Nurse within the last five (5) years, completion of specific free courses is required

Additional Required Qualifications:

• Ability to be on call and report to the site/client’s home within 30-45 minutes of call

Preferred Qualifications:

• Training in case management principles
• Knowledge of the continuing care sector and various levels of care
• Experience in wound care with excellent knowledge of wound care principles and product use
• Experience and training in Connect Care
