Your Opportunity:

Our client is dedicated to Enhancing Care in the Community, helping people be healthy, well, and independent in their homes and communities. The Continuing Care program offers services through five main areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life.

Job Responsibilities:

As a self-directed team member with Continuing Care, the Registered Nurse will:

  • Work collaboratively with the health care team.
  • Assess needs and deliver appropriate therapeutic nursing services within professional practice standards.
  • Coordinate resources, establish goals for client delivery, and evaluate outcomes.
  • Provide case management and direct client care meeting established clinical standards that are evidence-based and client-centered.
  • Act as a consultant on the team, providing clinical expertise and direction to other team members.
  • Develop care plans based on unmet needs using internal and external resources.
  • Participate in staff development and mentor student clinical experiences.
  • Support the Short-Term Intervention Team (STIT) with triage, clinic support, and community visits requiring specialized skills in cardiac, IV therapy, and wound care.

    • Description:

    As a Registered Nurse (RN), you will:

  • Provide a variety of nursing services to patients, families, communities, and populations while ensuring their safety and well-being.
  • Utilize nursing processes through critical thinking, problem solving, and decision making.
  • Teach, counsel, and advocate on behalf of patients and their families.
  • Support an integrated and holistic approach to patient care, health promotion, and maintenance.
  • Deliver safe, quality patient and family-centered care reflecting the shared vision and values of our client.

    • Work Environment:

  • Pay for Provisional Permit Holders: $39.58/hour – $54.16/hour.
  • Location: St. Marguerites Health Services B.
  • FTE: 0.70 (Part-Time).
  • Shifts: Days, Evenings, Nights, Weekends, On Call.
  • Requirements: Driver’s License and Vehicle Required.

    • Required Qualifications:

  • Completion of an accredited nursing education program.
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
  • If not practiced as a Registered Nurse within the last five years, completion of specific courses is required.

    • Additional Required Qualifications:

  • Two years of recent nursing experience or completion of a refresher program within two years.
  • Self-directed with the ability to work independently.
  • Excellent organizational and communication skills.
  • Ability to demonstrate computer knowledge effectively.
  • Physically capable of performing nursing tasks, including lifting and carrying medical bags weighing 25-30 lbs.

    • Preferred Qualifications:

  • Baccalaureate degree in nursing preferred, but relevant skills and experience in the nursing community sector will be considered.
  • Palliative care experience.
  • Previous Continuing Care and/or geriatric experience.
  • Experience with Connect Care and the RAI Home Care assessment tool.
  • At least two years of current Case Management Experience in Continuing Care.
