Our client is dedicated to enhancing care in the community, helping people be healthy, well, and independent in their homes and communities. The Continuing Care department in the Edmonton zone provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life.

Position Overview

As a self-directed team member with Continuing Care in the Edmonton zone, the Registered Nurse works collaboratively with the health care team. Responsibilities include:

  • Assessment of needs and delivering appropriate therapeutic nursing services within professional practice standards.
  • Coordinating resources, establishing goals for client delivery, and evaluating outcomes.
  • Providing case management as well as direct client care that meets established clinical standards, based on evidence-based and client-centered practices.
  • Collaborating with family and other partners, acting as a consultant on the team, providing clinical expertise and direction to other team members regarding client cases.
  • Developing care plans based on unmet needs using the appropriate internal and external resources.
  • Participating in staff development and mentoring student clinical experiences.

    • The position is based out of the Westview Home Care office but will require travel to other home care network offices as necessary.

    In your role as a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations while ensuring their safety and well-being. Key aspects of your role include:

  • Utilizing nursing processes through critical thinking, problem-solving, and decision-making.
  • Teaching, counseling, and advocating on behalf of patients and their families.
  • Playing a leadership role in supporting an integrated and holistic approach to patient care, health promotion, and maintenance.
  • Providing safe, quality patient and family-centered care while reflecting the shared vision and values of our client.

    • Salary Information

    Provisional Permit Holders shall be paid $39.58/hour – $54.16/hour. Upon being registered as a Registered Nurse, the rate of pay shall adjust according to applicable agreements.

    • Minimum Salary: $43.26
    • Maximum Salary: $59.21

    Required Qualifications

  • Completion of an accredited nursing education program.
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
  • If not practiced as a Registered Nurse within the last five (5) years, completion of specific free courses is required.

    • Additional Required Qualifications

  • Two years of recent nursing experience or completion of a refresher program within 2 years.
  • Ability to work independently and use sound professional judgment.
  • Excellent organizational, communication, and assessment skills.
  • Physically capable of performing nursing tasks which could include the transfer of clients.

    • Preferred Qualifications

  • Baccalaureate degree in nursing preferred, but skills and experience relevant to nursing practice in the community sector will be considered.
  • Palliative care experience.
  • Experience with Connect Care and the RAI Home Care assessment tool.
  • Two years of current Case Management Experience in Continuing Care.
